I am Dr Chandana Jayasundara attached to the Department of Obstetrics and Gynaecology of the Faculty of Medicine of the University of Colombo. My current designation is Senior Consultant in Obstetrics and Gynaecology and trainer of the PostGraduate of Medicine, University of Colombo. Hereby I am inviting you to take part in the research project titled A PERSONAL HEALTH RECORD FOR EVERY MOTHER BABY DYAD - A GROWTH ORIENTED PHR (GO-PHR) : AN IMPLEMENTATION AND FEASIBILITY STUDY conducted by Doctors Chandana Jayasundara, Ananda Perera, [INSERT YOURs and colleagues’ NAMES].The purpose of this research project is to assess the feasibility of implementing an electronic personal health record for Sri Lankan patients.
Your participation in this study is fully voluntary. You are free not to participate at all or to withdraw from the study at any time despite initial consent. By doing so you will not lose any medical care/treatments or lose any privileges to which you are otherwise entitled. If you decide not to participate or withdraw from the study you can do that at any stage of the study without you having to give any explanations for doing so.
You are recruited into this study by virtue of the fact that you responded to the awareness creation program through the print and social media.
By consenting to be a participant in this study you will be subjected to extra interviewing over and above that required for your routine medical care, your personal details will be stored electronically in cloud based computer systems and this study continues approximately for 1 year. You may receive SMS messages, alerts regarding appointments and drug refills if they are suggested by me when you were in the ward.
By participating in this study you may obtain the following benefits : documentation of all your encounters with the ward staff, reminders for follow ups, reminders for laboratory tests if ordered, reminders for any referrals if ordered, reminders for drugs refills, medicine reconciliation (you will get a list of all the drugs you are currently taking to be shown to any other doctor whom you may consult
By participating in this study you will not be subjected to any medical or surgical risks or hazards. The only risk for you is regarding the privacy and confidentiality of your personal data. As the principal investigator of the study I have taken all the necessary precautions for the security of your personal data. We followed the international guidelines on the security for personal health information, ministry of health security guidelines for personal health information, use of information and communication technology infrastructure which is always updated with security patches automatically, and also industry standard cyber security levels and documentation of our security policy at the technology office of the company in Colombo located at the no 12 Ridgeway Place, Bambalapitiya Colombo 4.
You will not receive any payments for your participation in this project.
We guarantee the confidentiality of all study records. Only anonymous data will be published and no information though which you may identified will be released. Published data will never contain any information traceable to you without your express permission. Data without identification information may be shared with other researchers as some journals expect the authors to make their data available to other researchers.
You may withdraw your consent to participate in this study at any time with no penalty or effect on medical care or loss of benefits. Please notify me as soon as you decide to withdraw your consent.

මම කොළඹ විශ්වවිද්‍යාලයේ වෛද්‍ය පීඨයේ ප්‍රසව හා නාරිවේද දෙපාර්තමේන්තුවට අනුයුක්තව සේවය කරන වෛද්‍ය චන්දන ජයසුන්දර. මගේ වර්තමාන තනතුර ප්‍රසව හා නාරිවේදය පිළිබඳ ජ්‍යෙෂ්ඨ උපදේශක සහ කොළඹ විශ්වවිද්‍යාලයේ වෛද්‍ය පශ්චාත් උපාධිධාරී පුහුණුකරු වේ. මෙය වෛද්‍ය චන්දන ජයසුන්දර සහ වෛද්‍ය ආනන්ද පෙරේරා විසින් පවත්වනු ලබන ක්‍රියාන්විතයේ සහ ශක්‍යතාවේ අධ්‍යයනයකි. මෙම පර්යේෂණ ව්‍යාපෘතියේ අරමුණ වන්නේ ශ්‍රී ලාංකේය රෝගීනට විද්‍යුත් පුද්ගලික සෞඛ්‍ය වාර්තාවක් ක්‍රියාත්මක කිරීමේ ශක්‍යතාව තක්සේරු කිරීම වේ.
මෙම අධ්‍යයනයට ඔබේ සහභාගීත්වය සම්පූර්ණයෙන්ම ස්වේච්ඡාවෙන් සිදු වේ. මුලික කැමැත්ත තිබියදීත්, කිසිසේත්ම සහභාගී නොවීමට හෝ ඕනෑම අවස්ථාවක අධ්‍යයනයෙන් ඉවත් වීමට ඔබට නිදහස ඇත. එසේ කිරීමෙන් ඔබට කිසිදු වෛද්‍ය ප්‍රතිකාර/ප්‍රතිකාරයක් අහිමි නොවනු ඇත හෝ ඔබට වෙනත් ආකාරයකින් හිමිවන වරප්‍රසාද අහිමි නොවේ. ඔබ අධ්‍යයනයට සහභාගී නොවීමට හෝ ඉවත් වීමට තීරණය කරන්නේ නම්, ඒ සඳහා ඔබට කිසිදු පැහැදිලි කිරීමක් ලබා දීමකින් තොරව අධ්‍යයනයේ ඕනෑම අදියරකදී ඔබට එය කළ හැකිය.
ඔබව මෙම අධ්‍යයනයට බඳවා ගනු ලබන්නේ ඔබ මෙම වාට්ටුවට ඇතුළත් කර තිබීම හේතුවෙනි.
මෙම අධ්‍යයනයට සහභාගී වීමට කැමැත්ත පළ කිරීමෙන් ඔබේ සාමාන්‍ය වෛද්‍ය ප්‍රතිකාර සඳහා අවශ්‍ය ප්‍රමාණයට වඩා වැඩි සම්මුඛ පරීක්ෂණවලට ඔබ යටත් වනු ඇත, ඔබේ පුද්ගලික තොරතුරු ගගන සන්චිත පරිගණක පද්ධතිවල ඉලෙක්ට්‍රොනිකව ගබඩා කෙරෙන අතර මෙම අධ්‍යයනය ආසන්න වශයෙන් වසර 1ක් පුරාවට සිදු වේ. ඔබ වාට්ටුවේ සිටියදී වෛද්‍යවරුන්
විසින් යෝජනා කරන ලද ප්‍රතිකාර සහ උපදෙස් ඔබට කෙටි පණිවුඩ, හමුවීම් සහ ඖෂධ නැවත පිරවීම පිළිබඳ ඇඟවීම් ලැබෙනු ඇත.
මෙම අධ්‍යයනයට සහභාගී වීමෙන් ඔබට පහත ප්‍රතිලාභ ලබා ගත හැක: වාට්ටුවේ කාර්ය මණ්ඩලය සමඟ ඔබ සිදු කල සියලුම වෛද්‍ය උපදේශන වල සියලුම ලිපි ලේඛන, පසු විපරම් සඳහා මතක් කිරීම්, ඇණවුම් කළහොත් රසායනාගාර පරීක්ෂණ සඳහා මතක් කිරීම්, ඇණවුම් කළහොත් ඕනෑම යොමු කිරීම් සඳහා මතක් කිරීම්, ඖෂධ නැවත පිරවීම සඳහා මතක් කිරීම්, ඖෂධ ප්‍රතිසන්ධානය. ඔබට උපදෙස් ලබා ගන්නාවූ වෙනත් ඕනෑම වෛද්‍යවරයෙකුට පෙන්වීමට ඔබ දැනට ගන්නා සියලුම ඖෂධ ලැයිස්තුවක් ලැබෙනු ඇත.
අපගේ ලේඛනගත කිරීම් වලදී අපි පුද්ගලික සෞඛ්‍ය තොරතුරු සඳහා වන ආරක්‍ෂාව, පුද්ගලික සෞඛ්‍ය තොරතුරු සඳහා සෞඛ්‍ය ආරක්ෂක අමාත්‍යාංශයේ මාර්ගෝපදේශ, ආරක්ෂක පැච් සමඟ සෑම විටම ස්වයංක්‍රීයව යාවත්කාලීන වන තොරතුරු සහ සන්නිවේදන තාක්ෂණ යටිතල පහසුකම් භාවිතය සහ කර්මාන්ත සම්මත සයිබර් ආරක්ෂක මට්ටම් පිළිබඳ ජාත්‍යන්තර මාර්ගෝපදේශ අපි අනුගමනය කළෙමු. අපගේ ආරක්ෂක ප්‍රතිපත්තිය පිලිබඳ මුද්‍රිත ප්‍රකාශනය කොළඹ 4, බම්බලපිටිය රිජ්වේ පෙදෙසේ අංක 12 හි පිහිටි අප සමාගමේ කොළඹ පිහිටි තාක්ෂණ කාර්යාලයේ තැන්පත් කර ඇත.
